- The Lateral Decubitus Position: Dr. Garcia shares that he is a huge proponent of placing the patient on their side (lateral decubitus position) for surgery rather than using a standard beach-chair position. He explains that many anterior (front) labral tears actually extend all the way around to the posterior (back) of the joint. Operating laterally makes it much easier to visualize these extensive tears and toggle smoothly between the front and back of the shoulder socket when unexpected damage is discovered.
- The Gemini Cannula as a Retractor: Accessing the back of the shoulder joint can be highly difficult because the capsule tissue is extremely flexible. To solve this, Dr. Garcia highlights his use of a specialized tool called the Gemini cannula. This cannula features built-in expandable "fingers" that act as an internal retractor, pushing the flexible capsule away to open up a clear, accessible workspace for the surgeon.
- Combining Advanced Tools: He notes that combining this retraction cannula with ultra-small knotless FiberTack anchors provides a major surgical advantage when navigating tight spaces in the posterior shoulder. He adds that this combination becomes even more valuable when a surgeon transitions into executing advanced accompanying procedures, such as a remplissage.
