Procedure Overview:
- Site Preparation: The surgeon begins by clearing and prepping the complex capsular tear and its accompanying small horizontal component to ensure an optimal environment for healing.
- FiberStitch Deployment: Multiple FiberStitches are placed through an all-inside approach. The design of these smaller 1.5 stitches allows for better tissue purchase without causing damage to the remaining healthy meniscus.
- Suture Tensioning: The implants are deployed in both horizontal and vertical mattress configurations (with stitches placed above and below the tear line). The surgeon carefully retensions the alternating suture limbs to maximize tissue approximation back to the joint capsule, ensuring all slack is removed.
- Fixation Strength: The surgeon shifts portals to place additional vertical mattress stitches. A structural pull-test demonstrates strong initial fixation with zero pull-out or tearing from the capsule walls.
- Marrow Stimulation: To conclude the repair, a microfracture is performed in the femoral notch. This deliberate poking of the bone allows bone marrow and native stem cells to egress into the joint space, which has been shown to significantly boost meniscus healing rates.
