- Self-Punching Lateral Row Fixation: Shifting to the lateral row, Dr. Garcia introduces all-PEEK (polyetheretherketone) self-punching SwiveLock anchors. Because these anchors can be driven directly through bone without pre-drilling or tapping a pilot hole, they significantly decrease surgical time and preserve local bone density.
- Intraoperative Dog-Ear Reduction: While tensioning the anterior lateral row, a tissue wrinkle or "dog-ear" becomes apparent at the tendon's edge. Dr. Garcia leverages the micro-adjustable, knotless mechanism of the SwiveLock anchor to precisely tension the loose margin, flattening the dog-ear completely down against the bone.
- Crossover SpeedBridge Finalization: The remaining suture limbs and tapes are tensioned across the footprint in a crisscross fashion. They are driven into a final posterolateral self-punching anchor, finishing a robust, double-row grid construct.
The Mechanical Result: The self-punching SpeedBridge construct provides maximum compression across the entire humeral footprint.
