In this video, Dr. Grant Garcia demonstrates his advanced arthroscopic stabilization technique for a patient suffering from chronic Multidirectional Instability (MDI) of the shoulder. The patient failed conservative, non-operative management over a five-year period and recently sustained a highly painful subluxation event. Arthroscopic examination revealed an extensive 360-degree labral tear coupled with global capsular laxity.

To address this severe, multi-planar laxity, Dr. Garcia executes a comprehensive, tailored reconstruction across the superior, posterior, and anterior zones of the shoulder joint:

Superior Repair (SLAP Component)

Operating with the patient in the lateral decubitus position, Dr. Garcia addresses the superior labral pathology first:

  • Preparation: The bony bed of the superior glenoid is carefully decorticated using an arthroscopic shaver and an aggressive burr to encourage biological healing.
  • Anchoring: Using an anterior-superior working portal, two 1.75 mm knotless FiberTack anchors are driven into the bone. Crucially, Dr. Garcia uses a specialized 15-degree angled guide to ensure the anchors dive safely into the bone without piercing through the thin glenoid wall.
