This video features Dr. Grant H. Garcia demonstrating an MPFL (medial patellofemoral ligament) reconstruction technique to treat recurrent patella dislocations.
The procedure utilizes a semitendinosus allograft and involves the following steps:
- Making a small arthrotomy in the superior portion of the patella to prepare the bone surface.
- Placing two suture anchors into the patella to recreate the previous anatomic MPFL.
- Identifying "Schöttle's point" under fluoroscopy to appropriately place a guide pin and reamer at the medial portion of the knee.
- Using suture tapes to ensure isometric tension before final reaming.
- Whip-stitching the allograft over the patella and securing it to the anchors with a tension slide technique.
- Passing the graft through the prepared tunnel between layers two and three, then securing it with a PEEK screw at roughly 45 degrees, taking care not to over-constrain the knee.
- Checking the patient's stability at multiple angles (0, 45, and 90 degrees) and performing a final arthroscopy to confirm the graft is properly positioned outside the joint space.
