Dr. Garcia demonstrates his inside-out meniscus repair technique for complex tears.

Here is the step-by-step description of the exact inside-out meniscus repair procedure demonstrated by Dr. Grant Garcia in his video for a bucket-handle meniscus tear:

  • Rasping the Base: The procedure begins by using a ball rasp to clean the base of the capsular meniscus. Dr. Garcia leaves the meniscus in its displaced "bucket handle" position during this stage, as it provides better stability and makes cleaning the tissue easier
  • Shaving the Capsule: A motorized shaver is introduced to further roughen up the tissue capsule. This intentional micro-trauma promotes localized bleeding once the surgical tourniquet is released, laying down an essential healing bed.
  • MCL Trephination: To improve surgical access inside the tight medial compartment of the knee, the surgeon performs trephination (making small punctures) on the Medial Collateral Ligament (MCL). This relaxes the joint space to allow safe suture passage without causing long-term negative effects to the ligament.
