This video features Dr. Grant Garcia demonstrating a highly advanced, open-boundary procedure for treating severe bipolar shoulder bone loss by combining a Distal Tibial Allograft (DTA) for the socket alongside a Humeral Head Allograft for the arm bone.
Bipolar bone loss occurs when a patient suffers from significant bony defects on both sides of the joint: an anterior glenoid bone loss defect (Bankart bone loss) and a massive, deep, structural Hill-Sachs lesion on the humeral head. When the bone defects on both sides are large, standard soft-tissue repairs (like a Bankart repair and a Remplissage) are insufficient because the shoulder will continue to slip and "engage" over the edges of the bone gaps. By performing an all-allograft anatomical reconstruction, Dr. Garcia physically replaces the missing bone on both sides to restore the perfect, rounded geometry of the entire ball-and-socket mechanism.
