In this video, Dr. Grant Garcia demonstrates his advanced, minimally invasive arthroscopic-assisted AC (Acromioclavicular) and CC (Coracoclavicular) joint reconstruction. This procedure is designed to restore multi-directional stability to a shoulder with a severe, chronic separation where both sets of stabilizing ligaments have completely torn.

The surgical reconstruction progresses through several key stages:

Arthroscopic Isolation & Skeletonization

  • Diagnostic Sweep: The procedure opens with a standard diagnostic arthroscopy from the posterior portal to inspect the inner shoulder joint.
  • Coracoid Clearing: Dr. Garcia moves into the subacromial space, utilizing a combination of a radiofrequency (RF) wand and an arthroscopic shaver to skeletonize the coracoid process. He clears the soft tissue off the lateral border and under-surface of the coracoid while pushing remaining tissues inferiorly. The RF wand is deliberately used as a blunt dissector to avoid any damage to nearby major neurovascular structures.
  • Prepping the Healing Bed: Dr. Garcia establishes an accessory anterior-inferior working portal with a large 8.25 mm cannula and switches to a 70-degree scope to perfectly visualize the bone landmarks. The under-surface of the coracoid is gently abraded with a shaver to create a raw, textured bone bed that will optimize biological healing of the soft tissue graft.
