- Overview of the Procedure The patient presented with an acute radial avulsion right off the root, which differs from a standard radial split tear.
- Sutureloc and Tunnel Preparation: The surgeon utilizes a Sutureloc guide with a 2.4 pin, carefully removing cartilage where the pin exits to protect the suture lock mechanism. The tibial tunnel is created while keeping the pin in place to avoid tunnel collision.
- Suture Management and Fixation: A nino wire, fiber link, and the suture lock are passed and appropriately tensioned. A horizontal mattress stitch is placed using a knee scorpion, followed by a simple stitch. The repair is partially tensioned initially to prevent damage during subsequent steps.
- Conclusion of ACL and Final Fixation: Once the ACL reconstruction and internal brace are completed, the lateral root repair is fully tensioned. The method provides excellent biomechanical strength with a reduced risk of slippage compared to traditional trans tibial tunnels.
